How to refer a client
We have attached a referral form for use with Commissioning Bodies, Direct Payments and Private Clients. Once completed one of our team will be in contact to confirm details and set up an introduction. This will allow us to meet with the client and agree outcomes and explain our service prior to sessions starting.
